Black Country Transition
Action Group
A multi-disciplinary group of professionals based in the Black Country and Birmingham, working with young people who have complex needs moving from children to adult services, coming together to support and achieve good transition. The group engage with likeminded professionals from all disciplines, working with young people with long term and life limiting illnesses and their families. We aim to learn from each other’s experiences, share best practice, hear from informative speakers and make useful links and contacts.
We have 34 members and have been meeting for five years. We have useful resources, keep in touch with national and local networks and wide-ranging professional interest to help with any problems you may have. We also distribute regular emails discussing what’s happening near and far, what’s new and what to look forward to with interesting opportunities to work together.
We can’t do it alone we must all work together to attain best results for young people and their families. If you would like to join the group or arrange to come and look around the Bridge and find out more. We are running virtual meetings when unable to get together at the Bridge.
